Twee vleugels van een drieluik. Op de binnenzijde van het rechter zijpaneel het portret van een knielende stichter vergezeld van Johannes de Doper. Op de binnenzijde van het linker zijpaneel een knielende stichteres met de heilige Adrianus (voor de buitenzijden en het middenpaneel zie SK-A-962-B en SK-A-962-A).
